1. Union-wide unique single identification number of the testing
2. Name and contact details of the provider or prospective provider and deployers involved
3. Brief description of the AI system, its intended purpose, and identification information
4. Summary of the main characteristics of the testing plan
5. Information on any suspension or termination of the testing
