The EU declaration of conformity shall contain all of the following information:
1. AI system name and type and any additional unambiguous reference allowing identification and traceability
2. Name and address of the provider or their authorised representative
3. Statement that the declaration is issued under the sole responsibility of the provider
4. Statement that the AI system conforms with this Regulation and any other relevant Union law
5. Where the AI system processes personal data, statement of compliance with Regulations (EU) 2016/679 and (EU) 2018/1725 and Directive (EU) 2016/680
6. References to harmonised standards used or common specifications
7. Where applicable: name and identification number of the notified body, conformity assessment procedure description, and certificate identification
8. Place and date of issue, name and function of signatory, signature
